This is not a play-action or RPO look, it is a straight run play with a built-in pass alert, so the primary read progression is pre-snap: check the alert to Z if you diagnose an overloaded box or man coverage on the outside, otherwise hand it off and let the FB lead into the hole with the HB reading the iso block for the crease.
Check the cornerback depth on the Z receiver pre-snap, if he is playing tight man coverage with no safety help over the top the comeback route can be jumped for a pick or incompletion. Use the alert check when you see the defense stacking the box with 7 or more defenders, since that signals man coverage on the outside and opens up the Z comeback for an easy explosive gain.
