This is a run play, not a play-action look, designed to attack gap integrity between the tackle and the tight end rather than bounce wide. The primary read progression is playside guard and tackle double team leverage first, then the TE's edge seal, with the HB reading whether to press the hole downhill or bounce outside if the edge gets pinched.
Watch the Sam linebacker or edge defender's alignment before the snap, if he is crashing hard inside pre-snap you get an easy bounce to the edge. Key the playside tackle and TE double team at the snap, if they can't generate movement the run dies at the line and you should check to a cutback or bail on the play call next series. This run beats light boxes and man coverage fronts that pull a linebacker or safety out for coverage responsibilities, leaving fewer bodies to fill run gaps. It gets shut down by stacked fronts with an extra defender in the box or aggressive user-controlled linebackers flowing downhill on the mesh point. Look for a user spy or lurking defender sitting in the B gap post-snap, since that indicates the defense is selling out on the interior run and you should bounce the play wide immediately.
Use this against defenses showing light boxes with six or fewer defenders in the box, since the double TE personnel invites base or nickel fronts that can't stack the edge. Check the pre-snap alignment of the playside Sam or edge defender, if he is squeezed inside you have a clean bounce read outside the tackle. Pair this with HB Dive or HB Slam Mtn out of the same 12 personnel to keep linebackers honest on downhill run fits before hitting Off Tackle to the edge. Run this on early downs, first and second and short to medium, when you want positive yardage without tipping pass intentions. Mix in HB Counter Str from the same formation to punish defenses that start flowing hard to the playside TE's edge seal after a few Off Tackle reps.
