Stacked fronts or heavy zone blitz packages that keep a linebacker home on the backside can shut this down quickly, especially if the defense diagnoses counter via down block angles. Look for safeties cheating into the box pre-snap, since an extra run defender crashing the counter timing forces the HB to bounce it outside into a spilled alley with no cutback lane.
Use this play when the defense shows a strong side overload or crashes hard toward the strength, since the counter action punishes linebackers flowing fast to the play call side.
Watch pre-snap for a heavy box (7 or more in the box) with linebackers creeping up, which tells you the misdirection will hold their eyes just long enough for the pulling blockers to seal the weakside edge.
