Inside Zone out of Shotgun Flex Y Off is a straightforward zone run play with the HB aligned next to the QB. The diagram shows the offensive line flowing laterally with zone blocking rules while the HB takes a downhill path off the RB's initial track, reading the frontside gap before cutting backside if it's clogged. This is a run play, not a pass or RPO look based on the art shown, and the primary read is the play side double team at the point of attack, with the backside cutback lane as the secondary read if the first and second level defenders flow too hard.
Key the middle linebacker's first step at the snap since his flow direction tells you which gap the ball is designed to hit. Watch for a stacked box with eight or more defenders, which signals the defense is selling out to stop the run and daring you to throw. If the defensive line is slanting hard to the play side, user your backside defensive tackle to squeeze the cutback lane and blow up the zone rules. Against man coverage on the perimeter, crash a safety down into the box pre-snap to add a extra run fit and take away the cutback. Beware of a mike linebacker blitzing downhill through the A gap right at the snap, since that nano-style blitz can beat inside zone blocking before the double team forms.
Use pre-snap motion or a hard count to see if linebackers declare their gaps before the snap, since Inside Zone lives or dies on how disciplined the front seven is. Attack light boxes with six or fewer defenders in the box, especially against nickel or dime personnel that leaves only two off ball linebackers. Hot route the TE to a quick chip or stay-in block versus heavy blitz looks so you keep an extra body at the point of attack. Pair this with RPO HB Zone Alert or RPO Alert HB Zone from the same set so the defense has to respect both the give and the pull read, punishing overaggressive linebacker flow. Best called on early downs (first and second and long) to keep defenses honest and set up play action shots like PA Mtn Slide or PA Seam Digs later in the series.
