The diagram shows four down linemen and the Mike crashing straight ahead with a nickel defender looping off the weak edge, while both outside corners and the deep safeties sit in press or off-man alignment with no deep help shown. This is a blitz designed to win with numbers and disguise pre-snap, betting that the extra rusher gets to the quarterback before the man coverage behind it breaks down.
Defensive Reads

Usage Tips
Call this on third and medium to long when you have already shown base Dime coverages so the blitz look catches the offense off guard
User the slot defender coming off the edge to time the blitz with the snap count and disrupt any quick game or hot route beaters
Check the pre-snap protection, if the offense slides protection to the blitz side, consider audibling to Dime Blitz 2 or 1 LB Dog instead
Pair this call in a sequence with Cover 1 Hole or Cover 1 Robber Press to keep the quarterback guessing between five man pressure and standard man looks from the same personnel
Avoid calling this into empty backfield sets since the extra blitzer has no gap advantage and the offense can pick it up easily with max protection
